You must include your E.I statement with every SUB card submission. Once you receive your first E.I payment, immediately submit your SUB card and E.I statement to the return address on the SUB card or via email to the address listed below. Please make sure to include the phrase “SUB Fund” in the subject line if sending via email. Submit your follow up SUB cards every time you receive subsequent E.I statements.
